Insurance
In-Network. Josh is provider of the following insurances:
- Health Choice
- Oklahoma Health Network
- Coventary/First Health Network
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Questions to Ask Your Insurance Provider
Out-of-Network. If you wish to use an insurance that is not listed above (example: Blue Cross Blue Sheild, United Healthcare, Cigna, or Value Options), you will first need to contact your insurance company and check on your out-of-network mental health benefits. If you have out-of-network coverage, you will be required to pay for your session in full; Josh would then be more than happy to submit the claim in effort to have your insurance company reimburse you directly.
Secondary Insurance. Insurance companies usually require that you use your primary insurance first before a secondary insurance will be of assistance to you. Please notify Josh of your primary insurance before your first session. Also, it may important for you to know that Josh does NOT file claims for secondary insurances.
Payment
Check is the preferred method of payment, but cash and credit/debit is also acceptable. All checks should be made payable to Family Solutions Counseling.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified your therapist at least 24 hours in advance, you will be required to pay the full cost of the session.
